Sculpting beauty with character
UK-educated equestrian-center owner transforms a round face into a refined V-line
Profile: MA in English literature, mother, equestrian-center owner. Diagnosis: ① Bulky lower face, thick fat layer and full cheeks. ② Poorly defined jawline. ③ Double chin especially visible when looking down. Plan: ① Minimally invasive liposuction through hidden 2 mm skin-line incisions behind the folds below both earlobes and at the chin-neck junction. ② Tighten skin while reducing lower-face volume, defining the jawline and improving the double chin for a younger appearance. ③ Pure fat removed: left 18 ml, right 13 ml, double chin 14 ml; total 45 ml.
“My face looked very full with no curve from the side, and my short chin made me feel round and heavy.”
After eight years in online consultation, a broad face becomes classically elegant, and she advances to lead in-person consultations
Profile: woman, 33, 160 cm/56.5 kg, mother and in-person consultation supervisor with eight years' prior online aesthetic consultation experience. Diagnosis: ① Square face, narrow forehead and temples, bulky lower face and pronounced double chin. ② Tear-trough and nasolabial hollows create an aged look. ③ Brow ridge, nose and chin lack projection. Plan: comprehensive facial subtraction and addition. ① Liposuction of the lower cheeks, jawline and double chin. ② Fat grafting to the forehead-temple region and within the hairline to improve outer and posterior contours. ③ Fat grafting to the brow ridge, apples of the cheeks, nose and chin for definition. ④ Fill tear troughs and nasolabial folds to reduce a tired appearance.
“I wasn't actually overweight, but my large face made my whole appearance look heavy.”
The patient who set Dr. Sang's personal facial-liposuction record
Profile: woman, 36, single, travel planner. Diagnosis: ① Obesity-prone build with a bulky mid-to-lower face and little separation between chin and neck. ② Cheek-fat accumulation, undefined jawline and pronounced double chin. Plan: ① Minimally invasive liposuction to reduce mid/lower-face volume, define the jawline and improve the double chin. ② Pure fat removed: left 58 ml, right 50 ml, double chin 15 ml; total 123 ml. ③ Because high-volume removal can loosen skin, a customized sharp needle stimulated collagen and elastic-fiber renewal in the superficial and dermal layers, supported by radiofrequency tightening.
“My face looked large and round, making me seem heavier and older.”
A subtle face gains definition through filling,a better version of herself
Profile: woman, 24, aesthetic-consultant assistant. Diagnosis: ① Visible tear troughs and nasolabial folds create an aged look. ② Short chin and undefined jawline. ③ Pronounced double chin. Plan: ① Fill the tear troughs, nasolabial folds, chin and temples for a smooth, full upper face, a V-shaped lower face and a clear jawline. ② Double-chin liposuction to reduce submental fullness.
“My temples were hollow and cheekbones high. People criticized my facial appearance, which really troubled me.”

Buccal fat removal
A streamlined treatment makes the face look smaller and slimmer while addressing a cause of facial aging.

What is deep buccal fat?
The deep buccal fat pad is a mass of fat beneath the superficial fat layer. It sits higher in the cheek until around age 18, then gradually descends with aging, contributing to sagging cheeks, jowl-like fullness and deeper nasolabial folds. In younger people it is also a major cause of facial roundness. Removing it can create a younger, smaller and slimmer face and, when performed early, may address an underlying cause of later cheek sagging.

Deep cheek tissue also descends with age.
Bone structure is not the only factor determining facial contours, so some concerns can be corrected without bone reduction. Effective procedures such as deep buccal-fat removal and facial liposuction can create smooth, natural contours.

Fat removal is performed through a 1–2 cm incision inside the mouth to reach the fat layer. Removing the right amount is essential, as excessive removal can leave the cheeks hollow.

Cheek liposuction (VASER)
Cheek liposuction (VASER)
This procedure suits excess cheek fat or loose, sagging skin around the cheekbones after facial contouring surgery. It improves an indistinct or drooping facial line, making the contour softer and smoother.
02

01
01
Steps
Make a small incision in the fold below the earlobe.
02
Inject tumescent solution into the treatment area for anesthesia and bleeding control.
03
Insert an ultrasound probe to selectively break down fat tissue.
04
Gently suction the disrupted fat and oil through a cannula, then close the incision and apply a compression dressing.

01
01
Targets fat tissue selectively
A VASER ultrasound cannula selectively disrupts fat while protecting nerves and blood vessels; the broken-down fat and oil are then removed through a fine tube.
02
Smooth facial contours
Because fat is carefully and evenly emulsified before suction, irregular skin texture is less likely.
03
Fast recovery
Minimal vascular injury reduces swelling and bruising, supporting faster recovery.
04
Minimally invasive, with little concern about visible scars
The procedure uses discreet openings in the earlobe and chin creases, so scars are barely visible.

Questions and answers
Questions and answers
Where are the incisions, and when are sutures removed?
It depends on the procedure. Double-chin treatment uses an incision below the chin; jawline contouring uses one behind the ear; deep buccal-fat and cheek procedures use an intraoral incision. Sutures are generally removed within one week.
